I'll be adding various forms here that you may find useful to complete ahead of your appointment. All forms are PDFs and many of them you will be able to fill out by opening in Adobe Acrobat and typing in the appropriate fields or simply clicking where you need to check.
The Minor Release Form grants permission for me to work on clients under the age of 18. The parent or guardian signing the form MUST bring it in before the first session and sign it in my presence. You are also welcome to stay for the session.
At this time the insurance forms only apply to clients covered for injuries in auto accidents and workmen compensation claims.
